Large randomised trials in adults with type 2 diabetes and in adults with obesity have reported reductions in body weight and improvements in several cardiovascular risk markers. One outcome trial found a lower incidence of major adverse cardiovascular events in participants with diabetes and established cardiovascular disease. Gastrointestinal effects such as nausea and vomiting are the most frequently reported adverse events and often diminish over time. Changes in lean body mass during weight loss are an area of ongoing investigation. Effects in adolescents and in pregnancy are less well characterised, and current labelling advises against use during pregnancy.

Organized crime refers to groups of centralized enterprises that engage in, and aim to profit from, illegal activities. Some forms of organized crime exist simply to meet demand for illegal goods or to facilitate trade in products and services banned by the state, such as illegal drugs or firearms. In other cases, criminal organizations force people to do business with them, as when gangs extort protection money from shopkeepers. In the United States, the Organized Crime Control Act (1970) defines organized crime as "[t]he unlawful activities of [...] a highly organized, disciplined association [...]". Criminal activity as a structured process is referred to as racketeering. In the UK, police estimate that organized crime involves up to 38,000 people operating in 6,000 various groups. Historically, the largest organized crime force in the United States has been Cosa Nostra (Italian-American Mafia), but other transnational criminal organizations have also risen in prominence in recent decades. A 2012 article in a U.S. Department of Justice journal stated that: "Since the end of the Cold War, organized crime groups from Russia, China, Italy, Nigeria, and Japan have increased their international presence and worldwide networks or have become involved in more transnational criminal activities.
== Epidemiology == Calciphylaxis most commonly occurs in patients with end-stage renal disease who are on hemodialysis or who have recently received a kidney transplant. When reported in patients without end-stage renal disease (such as in earlier stages of chronic kidney disease or in normal kidney function), it is called non-uremic calciphylaxis by Nigwekar et al. Non-uremic calciphylaxis has been observed in patients with primary hyperparathyroidism, breast cancer (treated with chemotherapy), liver cirrhosis (due to hazardous alcohol use), cholangiocarcinoma, Crohn's disease, rheumatoid arthritis (RA), and systemic lupus erythematosus (SLE). Calciphylaxis, regardless of etiology, has been reported at an incidence of 35 in 10,000 dialysis patients per year in the United States, 4 in 10,000 patients in Germany, and less than 1 in 10,000 patients in Japan. It is unknown whether the higher incidence in the United States is due to genuinely higher incidence or due to underreporting in other countries. Annual incidence in kidney transplant patients and in non-uremic calciphylaxis patients is also unknown. The median age of patients at diagnosis of calciphylaxis is 60 years and the majority of these patients are women (60-70%). The location of lesions, central (located on the trunk) or peripheral (located on the extremities), is dependent on several risk factors. Central lesions are associated with younger patients, patients with a higher body mass index, and a higher risk of death than those who have peripheral-only lesions.
The two substrates of this enzyme are carnitine and oxidised nicotinamide adenine dinucleotide (NAD+). Its products are 3-dehydrocarnitine, reduced NADH, and a proton. This enzyme belongs to the family of oxidoreductases, specifically those acting on the CH-OH group of donor with NAD+ or NADP+ as acceptor. == Histidine-rich metal-binding proteins == Poly-histidine tags (of six or more consecutive His residues) are utilized for protein purification by binding to columns with nickel or cobalt, with micromolar affinity. Natural poly-histidine peptides, found in the venom of the viper Atheris squamigera have been shown to bind Zn(2+), Ni(2+) and Cu(2+) and affect the function of venom metalloproteases. Furthermore, histidine-rich low-complexity regions are found in metal-binding and especially nickel-cobalt binding proteins. These histidine-rich low complexity regions have an average length of 36 residues, of which 53% histidine, 23% aspartate, 9% glutamate. Intriguingly, structured domains with metal binding properties also have very similar frequencies of these amino acids that are involved in the coordination of the metal. Accordingly, it has been hypothesized that these metal-binding structured domains could have originated and evolved/optimized from metal-binding low-complexity protein regions of similar amino acid content.
=== Intravenous glucose === If a person cannot receive oral glucose gel or tablets, such as the case with unconsciousness, seizures, or altered mental status, then emergency personnel (EMTs/Paramedics and in-hospital personnel) can establish a peripheral or central IV line and administer a solution containing dextrose and saline. These are normally referred to as Dextrose (Concentration) Water, and come in 5%, 10%, 25% and 50%. Dextrose 5% and 10% come in IV bag and syringe form, and are mainly used in infants and to provide a fluid medium for medications. Dextrose 25% and 50% are heavily necrotic due to their hyperosmolarity, and should only be given through a patent IV line – any infiltration can cause massive tissue necrosis. It is MUCH safer to use a Dextrose 10% solution when treating hypoglycemia via IV in children under the age of 14. When using Dextrose 25% in a child it is safer to administer it through a central line or an intra-osseous line.

== Prognosis == There are risks of long-term neurological effects from maple syrup urine disease. These may include ADHD, anxiety, depression, etc. The severity of these effects is dependent on the level of metabolic control. If left untreated, MSUD will lead to death due to central neurological function failure and respiratory failure. Early detection, protein-restricted diet low in branched-chain amino acids, close monitoring of blood chemistry, and aggressive treatment during a metabolic crisis can lead to a good prognosis with little or no abnormal developments. Early diagnosis can improve cognitive development, but the effects are largely dependent on the quality of healthcare systems and disease type. Cognitive development may be shown to be below that of the general population. The severity of the cognitive delay is related to the time the condition remained undiagnosed and the effectiveness of dietary control including during metabolic crises.
Venous insufficiency is the most common disorder of the venous system, and is usually manifested as either spider veins or varicose veins. Several treatments are available including endovenous thermal ablation (using radiofrequency or laser energy), vein stripping, ambulatory phlebectomy, foam sclerotherapy, laser, or compression. Postphlebitic syndrome is venous insufficiency that develops following deep vein thrombosis.
Trials are investigating whether botulinum toxin, when used to make a person appear to frown less, stops negative feedback from the face and affects depression. Psilocybin may have a beneficial role in the treatment of depression. A 2019 meta-analysis found that hypnotherapy may be an effective way of alleviating the symptoms of depression. No model of depression in animals that fully explains the mechanism of depression has been found as of 2019.

On 23 February 2024, the director of Doctors Without Borders reported, "Psychological injuries have led children as young as five to tell us that they would prefer to die." Save the Children found, "Children in Gaza are going through a period of mass-scale shock and grief. This war and the physical and mental scars it is leaving on children is further eroding their resilience." In July 2024, Julia Wendt, the emergency child protection coordinator with the International Rescue Committee, stated children in Gaza were suffering from nightmares and bedwetting due to the mental toll of the war. === Neuromuscular adaptations === Strength training is not only associated with an increase in muscle mass, but also an improvement in the nervous system's ability to recruit muscle fibers and activate them at a faster rate. Neural adaptations can occur in the motor cortex, the spinal cord, and/or neuromuscular junctions. The initial significant improvements in strength amongst new lifters are a result of increased neural drive, motor unit synchronization, motor unit excitability, rate of force development, muscle fiber conduction velocity, and motor unit discharge rate. Together, these improvements provide an increase in strength separate from muscle hypertrophy. Typically, the main barbell lifts – squat, bench, and deadlift – are performed with a full range of motion, which provides the greatest neuromuscular improvements compared to one-third or two-thirds range of motion. However, there are reasons to perform these lifts with less range of motion, particularly in the powerlifting community. By limiting range of motion, lifters can target a specific joint angle in order to improve their sticking points by training their neural drive. Neuromuscular adaptations are critical for the development of strength, but are especially important in the aging adult population, as the decline in neuromuscular function is roughly three times as great (≈3% per year) as the loss of muscle mass (≈1% per year). By staying active and following a resistance training program, older adults can maintain their movement, stability, balance, and independence.

The esophagus (American English) or oesophagus (British English) ( ), also known as the gullet, is an organ in vertebrates through which food passes, aided by peristaltic contractions, from the pharynx to the stomach. The esophagus is a fibromuscular tube, about 25 cm (10 in) long in adult humans, that travels behind the trachea and heart, passes through the diaphragm, and empties into the uppermost region of the stomach. During swallowing, the epiglottis tilts backwards to prevent food from going down the larynx and lungs. The word esophagus is from Ancient Greek οἰσοφάγος (oisophágos), from οἴσω (oísō), future form of φέρω (phérō, "I carry") + ἔφαγον (éphagon, "I ate"). The wall of the esophagus from the lumen outwards consists of mucosa, submucosa (connective tissue), layers of muscle fibers between layers of fibrous tissue, and an outer layer of connective tissue. The mucosa is a stratified squamous epithelium of around three layers of squamous cells, which contrasts to the single layer of columnar cells of the stomach. The transition between these two types of epithelium is visible as a zig-zag line. Most of the muscle is smooth muscle although striated muscle predominates in its upper third. It has two muscular rings or sphincters in its wall, one at the top and one at the bottom. The lower sphincter helps to prevent reflux of acidic stomach content. The esophagus has a rich blood supply and venous drainage.
